Sjögren's syndrome presenting as remitting seronegative symmetric synovitis with pitting edema (RS3PE): comment of the article by Choi et al.

چکیده
the authors reported on a 35-yr-old woman presenting as remitting seronegative symmetrical synovitis with pitting edema (RS3PE) and, three months later, complaining of dry mouth. The autoantibody profile, Schirmer’s test and minor salivary gland biopsy were compatible with Sjogren’s syndrome (SS). Therefore, the authors diagnosed SS presenting as RS3PE, and affirmed that there has been no previously published report of RS3PE in a patient with SS (1). Actually, the case observed by Choi et al. is not the first reported in the literature. In fact, in 2000 we described a 74-yr-old man in whom RS3PE antedated the clinical and immunological features of primary SS by several months (2). Moreover, in 1995 Schaeverbeke et al. (3) reported on a case of RS3PE preceding SS, in 1997 Tanaka et al. (4) on a case of RS3PE complicated with SS, and in 1999 Cobeta Garcia et al. (5) diagnosed SS in one of eight patients with RS3PE. Choi et al. (1) suggest that SS should be taken into consideration for the differential diagnosis of patients with RS3PE. We agree the assertion that SS has to be included in the list of the different diseases whose RS3PE may be a presenting manifestation (3).

Remitting seronegative symmetric synovitis with pitting edema (RS3PE) syndrome is characterized by symmetrical and acute synovitis, pitting edema, the absence of rheumatoid factor, increased acute phase reactants, lack of bony erosions on radiography, and benign and short clinical course. Half of all patients with Sjogren's syndrome experience arthritis during the disease course. We here descri...

Remitting Seronegative Symmetrical Synovitis with Pitting Edema (RS3PE) is an uncommon elderly-onset rheumatic picture descripted for the first time by McCarthy et al. in 1985 [1] characterized by tenosynovitis of extensor tendons at the wrist and (less frequently) at the feet. The edematous imbibition of tendon sheaths determines the “boxing-glove" swelling which can be considered when bilater...

Remitting seronegative symmetrical synovitis with pitting edema (RS3PE) is a syndrome that may be associated with many conditions, including malignancy. Three further cases of paraneoplastic RS3PE are described and the literature is reviewed. Paraneoplastic RS3PE is more frequently associated with solid tumors, in particular adenocarcinoma. The two clinical characteristics suggestive of paraneo...
